So in your scenario, insurance = tax. The only difference is the organisations like the NHS, education and the police would be run by profit making insurance companies. That was an assumption on my part, however it is a pretty safe assumption as I couldn't see many private businesses wanting to take up the hassle of running a health service without making a tidy profit.
