Gears slipping up or down the cogs?

You need to adjust the little barrel at the end of the cable, where it enters the derailleur. The direction depends on which way the gears are slipping -- do a quarter turn in one direction, test ride and see what happens. If it gets better you are going the right way, if not turn in the other direction.

 

If your wheel brushes the forks, either it is not properly centered and/or you have loose spokes. I have mine done at a proper bicycle wheelwrights with his own specialist shop. Places like Halfords won't take the same kind of care. My wheelwright isn't cheap but it's solid work and probably costs less in the long run because I don't go in three times a year. You should also get the brakes looked at, sounds like they might also be pulling off centre.

 

Short version, take it in to a reputable shop and have it inspected -- an independant is best because they actually care about cycling and will give an estimate beforehand. Places like Halfords will tighten a few bolts, slap a bit of oil on the chain and give you the bill.

 

At the end of the day, what's your safety worth? Would you rather pay someone who knows what they are doing a bit more, or some kid who gets a spoke key pushed into their hand and pointed in the direction of your bike?
