cgksheff
A glass may have an inherent weakness from manufacture. Cooling the molten glass too quickly can result in stresses remaining in the item and there may be a microscopic fracture there from the beginning. When a microscopic fracture starts, it will get bigger over time until it is large enough to cause the catastrophic shattering. A perfect glass will not develop cracks etc. from washing/cooling cycles alone.

verona
I used to sell glasses and watched the manufacturing process. When finished, the glass is constantly under immense stress and pressure. This to to make sure it does exactly as it did, shatter into loads of little pieces like a car windscreen. It could have been a number of factors, but if doubt a ghost is one of them. Saying that, we had thousands and thousands of them in stock and never once did I see one break by its self.
