It’s because far too many drivers, when faced with a choice of two lanes, choose the right hand one. 
 

And then forget that some of us prefer to drive in the correct lane😁

IF you can't answer this one correctly then maybe driving isn't for you. 

Lane 1 splits into 2 for the gyratory allowing traffic to either continue straight or turn right(not really a roundabout). Lane 2 turns right only. 

Anyone crossing from Lane 2 in to Lane 1 and causing an issue is at fault. They changed lanes.
